The Admission Process for In-Residence Services
Most levels of the in-residence care we offer have the simple three-step admission process shown below. Others, such as Fountainview at Eisenberg Village, may provide their own admission process. We are here to help and guide you through any admissions process for the level of care that is right for you.
Personalized Campus Tour
We welcome applicants and families to tour our campuses. Please allow about 90 minutes for the tour; Click here to request a tour.
After exploring the campus, we will provide you with an Application Packet.
Interview & Assessment
Once we receive your completed Personal Information Form and Physician's Report, we will reach out to you to assess your specific needs. You can meet with a Los Angeles Jewish Health nurse and Admissions Financial Coordinator. (Please note: Income levels do not determine admission to Los Angeles Jewish Health.)

After all admission criteria have been met, you can select among available rooms.
